若い方なのですが、歯槽膿漏で上の歯全体がグラグラされている方がご来院されました。
ご本人も歯の悪いことを自覚しておられ、初めからインプラントを希望されておられました。
どの様に治療を進めていくのか、患者様と十分に相談しました。あまり噛めなくても、無理矢理に
たった1年だけでも歯を残していくのか、それとも、学問的にダメな歯は諦めて抜歯し、インプラントを入れるのか・・・
相談の結果、患者様はインプラントを入れることを希望されました。確かに今以上に歯槽膿漏が進むと、今の状態でも
インプラント手術が大変であると予想されるのに、時間がたてばより一層大変な手術になることは想像に難くありません。
私は常に、治療に選択肢のある場合は、自分の親や兄弟ならばどの様な選択をするかを考え、患者様と治療方針を考えるようにしております。
今回のケースは、私も迷わずインプラント治療を選択します。
治療が始まりました。
まず、他院での治療、無理矢理ほとんどの歯を繋げていた物を全て外しました。何と驚くことに、全ての歯がグラグラに揺れていました。
結果残念なことに、全ての歯を抜くことになりました。もう少し早くにインプラント手術をしていたら、多くの歯が残っていたと思います。
抜歯の後、少しの間だけ入れ歯を入れて頂き、シンプラントを埋入する計画をたてました。
出来るだけ、入れ歯を入れている時間が少ないように、シンプラント埋入後、その日のうちに歯を入れるという
即時負荷という方法で手術を行なう予定です。
レントゲン上では歯は残っているのですが、かわいそうなぐらい揺れている状態でした
CTを撮影すると 骨が大変薄く、少し難しい手術になりそうです。
CTを撮影することで、頭蓋骨を３D(グラフィック）で診ることが可能です。
術前にシュミレーションで確認することにより安全により手術が行えます。
